Colorado Birth Certificate – Gender Change and/or Name Change:
  • Colorado Birth Certificate Gender Marker Information
  •  For Gender Change be sure to include a Court Order – links to forms on above site (Note: Colorado State Law CRS 25-2-115 requires Changed Birth Certificates to be marked β€œamended” and apparently requires a name change along with a court order of gender change to amend the gender)
  • For Both Name and Gender Change be sure to include Birth Certificate Correction Form, links included on above site
